When topping up, is the price paid on top of the original plan?+
Yes, a top-up charge is separate from the price already paid for the original plan. The price of each top-up option is shown clearly before you confirm it.
If the profile has already expired, can it still be topped up?+
Generally not, since topping up only works on a profile that is still active. Once it has expired, a separate new plan needs to be bought instead. Check the order page to see whether a top-up option still shows before assuming either way.
